4. SMS & Text Messaging Terms

Program Description: Central Medical Center offers SMS text messaging to patients and prospective patients who provide their mobile number and give consent on our forms. Marketing consent is collected separately from non-marketing consent. Our forms present two independent consent checkboxes, neither of which is pre-selected or required to submit the form:

Non-marketing messages: appointment confirmations, appointment reminders, follow-up care instructions, and account or billing notifications.

Marketing messages: promotional offers, wellness information, new service announcements, and special pricing.

You may consent to either, both, or neither. You will only receive the message types you have expressly agreed to receive. Consent to receive text messages is not a condition of purchase or treatment.

Message Frequency: Message frequency varies based on your appointments and the consents you have selected.

Message and Data Rates: Message and data rates may apply for any messages sent to you from us and to us from you. If you have questions about your text or data plan, contact your wireless provider.

Opt-Out: You can cancel the SMS service at any time. Reply STOP to any message you receive from us. We will send a confirmation message, after which you will no longer receive SMS messages from us. To rejoin, sign up again as you did the first time.

Help and Support: Reply HELP to any message for assistance, or contact us directly at GHL NUMBER or info@centralmedcenter.com.

Age Restriction: You must be 18 years of age or older to participate in our text messaging program.

Carrier Liability: Carriers are not liable for delayed or undelivered messages.

Mobile Information Sharing: No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Protected Health Information: Text messages are not a secure channel. We limit the health information included in SMS and will not transmit detailed diagnoses, lab results, or treatment records by text. For anything involving your protected health information, we will direct you to a secure channel or ask you to call our office.
